Keystone State Tournament Single Entry

This purchase is good for a single entry for the tournament. Includes sparring and forms. Please enter the participants name when checking out. (Tiny tots ages 5-6 years old will be entered in as 10th Gups.)
(Example: John Smith/ 6 year old/  10th gup)

1. ADULT WAIVER AND RELEASE OF LIABILITY.

2. MINOR WAIVER AND RELEASE OF LIABILITY.

In consideration of being allowed to participate in any way in Summit Martial Arts, martial arts activities, tournament, athletics/sports program, and related events and activities, the undersigned:

1. Agrees that the parent(s) and/or legal guardian will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate.

2. I am fully aware of my personal medical condition, and hereby certify that I am mentally and physically fit to participate in the Events, and that I have the medical coverage or personal means to cover the [any] expenses. I agree to be responsible for any and all costs and expenses including, but not limited to, medical expenses, resulting from injury to me. I acknowledge that the wearing of appropriate personal safety equipment, such as protective mouthpiece, groin cup and head protective gear, are basic safety precautions and that I am responsible for providing such equipment for my use. I agree to waive all claims against any persons connected with these Events for injuries I may sustain, and likewise assume full responsibility for all my actions in connection with these Events.

3. I acknowledge and fully understand that each participant will be engage in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own activities, inactions or negligence, but the actions or negligence of others, the rules of play, or the conditions of the premises or any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.

4. I assume all the forgoing risk and accept full responsibility for the damages following such injury, permanent disability or death.

5. I release, waive, discharge and convenant not to sue United States Soo Bahk Do Moo Duk Kwan or Summit Martial Arts, their administrators, directors, agents, coaches, other employees or any volunteers of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, all of which are hereinafter referred to as “releases”, from any and all liability, to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or part by the negligence of the releases or otherwise.

6. I understand that all fees paid to Summit Martial Arts in connection with this Event are non-refundable.

7. I permanently waive any compensation for photos, videos, media coverage, etc. of me that may be utilized by the United States Soo Bahk Do Moo Duk Kwan Federation and or Summit Martial Arts, and give permission for publication of them.

WE HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANCIAL RIGHTS
